Dr. Shk. Alfred Nona, cardiologist was invited this Tuesday in the show "Morning Abc" where he talked about pulmonary embolism, its causes and risks. Dr. Nona explained that pulmonary embolism is one of the leading causes of mortality and with Covid-19, the numbers have increased.
Pulmonary embolism can be fatal due to blockage of blood flow, caused as a result of blood clotting. Dr. Alfredi points out that timely treatment significantly reduces the risk of death. The doctor also pointed out that many patients with Covid-19 suffer from pulmonary embolism.
"In terms of prognosis, pulmonary embolism had a mortality rate of around 11 percent before the pandemic, the figures are higher now. A proportion of patients, 1/3 of them lose their lives without reaching the hospital or when they arrive and the treatment has not been done yet, when there was no time for the treatment to work, it reaches up to 15 percent of global pulmonary embolism mortality.
Case verification has increased dramatically because diagnostic capacity is many times higher. Mortality has been declining, it was higher 20 years ago but it is declining. When we talk about blood clots, we have to thaw this clotted blood and we have to use anticoagulants. Based on the high risk mortality classification, medium or low we give therapy.
A patient at high risk for acute mortality or within 30 days use thrombolytics, blood thinners. "We use different medications based on the onset of thrombosis," said Dr. Alfredi adding that surgeries are rarer as therapies usually work.
Further, Dr. Nona explained how pulmonary hypertension is caused. "If a blood clot is not thawed properly, it narrows the blood vessels and fibrifies. Does not dissolve with diluents and gives chronic thromboembolic pulmonary hypertension. This type is the only one that can be cured and a surgical intervention is performed on it.
Hypertension is important because it shortens the life of the patient if not treated properly. It is identified on the basis of an echocardiogram, with a scanner to see if there is any blood clot left and it has not been cleaned properly. The right heart is then catheterized and it is decided whether to clean the blood vessels. ", Explained Dr. Alfredi.
